As unlikely as it might be...an infection may have been introduced. I mean if it happened once in a thousand toppings...you may have just gotten unlucky. Maybe someone knows of a systemic agent that will cure that. Sorry...no clues.

Yes and the silica will also do it's other things. Stronger stems and such. I only grow indoors and haven't dealt with bugs much so don't have an insecticide to reccomend...but good luck with that.

That's just bad luck. There are a bunch of ideas floating around out there to encourage females. Most of it sounds like bro-science...but there could some truth too it. Might be worth a try.
Actually fems aren't that bad. I got some via a seed seller mistake...and they are growing nicely...

Are you using silica at all. I understand that silica applied as foliar spray can change leaf ph and discourage insects and powder mold. Totally safe...something to consider.

I remember several guys who set up rooms with parabolic shape. It was longitudinal with each side wall meeting in the center. Then they mounted a bare bulb at the loci of the parabola so that all reflected light is directed straight downward.
