See if it perks up more. Roots dying in air at the bottom perfectly normal. Roots at the top mean it's filled out the soil somewhat. Wait it out see if it gets its health back before anything then yes you can report.

White walls are very reflective, some tents are white inside not foil, both work. Your light looks very high, that seed might not be dead just not had long enough. I would be blowing air out, that light shouldn't make it too hot. Exhausts work that way.

Roots find nutrients why your seeing more, there will still be plenty left and root tips haven't filled all the soil yet. Water meters don't work, get use to judging yourself. Sometimes they droop after a water when they are thirsty then straighten up after.
