I finally recieved a response to my email today (I wanted to make sure the documents they said were missing were recieved) my app has been approved and card is at the printer! Finally a light at the end of the tunnel... :-)

It was a letter saying that they didnt recieve the dr consent form... blows my mind because I submitted online and filled in every blank. I submitted it via email so hopefully it gets corrected quickly!
