It was mailed. Dr. Said it had to be mailed in an envelope with the return address of his clinic It makes me wonder-am I his first rodeo . So if I call him tomorrow I can tell him to send it electronically?

Received letter today saying they have no certification from my dr. can he resubmit this to them in an e-mail or does he have to mail it. My app. Was received on 3/10 & my check was cashed 5/1. I am not a happy camper
