Heard back from IDPH and physician letter missing with application fml. Had to have my physician resubmit the form. Hoping some better luck for you guys. Peace!!

Application sent on week of May 21st. Contacted IPDH this morning and still in queue.

Submitted around May 22nd. Called yesterday still not reviewed. Should be any day now!
