Thanks...how does the lab know which samples have a prescription? Does the person who the test is being administered on provide this information, or does the lab check against some state level database?

hi all - just discovered this blog. here is the summary of my status so far: mailed in a paper application on 5/29/18 and check cashed on 8/7/18 (70 days after mailing application from Chicago). emailed DPH this morning to get a status, but no response yet.
