cost to become caregiver

i was just wondering how much the fees and costs for background checks would be or if anyone can point me in the right direction. any help would be awesome

$200 per patient, up to 5 patients. (they have to be 25+ miles as the bird flies from a dispensary)

First you must find a qualifying patient with a need and agrees to have you as their designated caregiver through the AzDHS. Once you have that figured it is easy!

Next you will need your patient to go to the AzDHS site and click Adult Patient Online Application and under the Other Option tab you will find 'Card Change Application' - (AzDHS website) http://www.azdhs.gov/medicalmarijuana/patients/adult.htm

Once your patient recieves an approval from the AzDHS then and only then you will get able to fill out a Degisanated Caregiver New Application found here too -

But before you submit your CG (caregiver) application you need a few items - Copy of State ID, Signed attestation, current photo, 2 set of finger prints & a $150 fee. Once you submit your CG application you will get a response from AzDHS - once approved then you may help your patient (acquire meds so on..)

NOTE
Unfortunately RIGHT NOW the AzDHS 'Change Card Application' is not working and you must have your patient add you through email - m2programsupport@azdhs.gov
