medical disclaimer for doctor

I know I saw this mentioned somewhere, in some thread, but I'll be damned if I can find it now.


Does anyone know where I can find this release form to offer reluctant doctors?

"The Canadian Medical Protective Association (CMPA) has developed a "Release Form for Medical Practitioners". This form is available on the CMPA website http://www.cmpa-acpm.ca. This is not a Health Canada form and completing the form is not a requirement to obtain an authorization to possess marihuana for medical purposes. Any questions relating to this form should be discussed with your physician."

Many of the medicinal cannabis dispensaries have downloadable release forms for your doctor if you are applying for membership.

that looks like at attempt at a plug for some compassion club, nothing there about doctor release forms, and the potential mmar patient has no interest (that I know of) in becoming a member of a compassion club in vancouver
