Special caregiver form from new law?

I have been told that as a caregiver in Colorado I need to fill out and send in this new form to be legal in the state.

However I cannot find this form anywhere, can someone send a link to this new form?

There is no new form, yet. The CDPHE will post on their site if/wwhen any new forms are available.
