what an odd OP, but it could be a lot of things lol. a great rip usually pulls my balls up a lil, but probly due to coughing, HOWEVER it could be more serious stuff like, torsion, or other things that are exacerbated by your coughing so much, because im guessing that its due to the way you breathe while your smoking, not the smoke itself, but idk.
