Whats wrong with this plant?

Leaves falling off as it grows. I used too many nutrients and so I tried to save it be changing the water. It's been in fresh water only for 2 weeks and it's still dieing.

Do I understand correctly, it's been in water only for 2 weeks? No nutes? Might b a thing, I dunno
