Update: Did a full dose Azamax drench yesterday. I can confidently say I see about 75% activity in my samples today than I did 2 days ago. Going to wait 5 days and hit it one more time. That should then give me 4-6 weeks of no treatment before harvest. Thanks for the ID help guys..

Actually I was trying to take better pictures and it looks like I have 2 pests.. or they are the same pests different stages. Instead of a worm / leech like.. this second one has legs, face, etc. here are a couple more pics. all in soil. nothing on plants.
