I would suggest more a magnesium deficiency - (yellowing of whole leaf, severe interveinal chlorosis) Below is a calcium deficiency i had recently. (leaf is still green but more necrotic spotting)

Yes. It is a biological answer to fungus gnats. Bacillus thuringi-ensis: subspecies israelensis. (B.t.i.). B.t.i. is a bacterium that is deadly to mosquito larvae but harmless to other living things. Safe for soil. (Never used this product myself)

How old is your mix..? How many times recycled? Might still be not quite available or broken down yet? I amend my ROLS with rock dust (among a few other things) at the end of harvest. Thatz all the 'known' silica i add. My plantz seem ok.

Basalt Rock dust has a shit load of Silicon dioxide (Silica) in it. Because of adding this i dont give it a second thought. Your Azomite supplement will have it in spades! Like upwardz of 50% of the total mineral makeup. Definitely the most abundant mineral in it. :)
