I water twice a week sometimes three depending, in the first picture you can see the part that fell off and dried up. It looked like it had gotten cut off as if someone tried to "top" the plant and left the new node and growth laying there who knows perhaps I did it in my sleep. Still confused...

Could possibly it gotten Fucked up by my girlfriend touching it she has a habit of doing that...

The stem is just black looking not hollow almost like it got burnt from the light that's what I thought

So is it Fucked or can I fix it, how would I have gotten this issue I. The first place?
