to become a caregiver, if u have a card already, do u need to fill something out and get approved or just send in ur patients paper work.

your patient needs to be accepted by LARA as a patient either with you as caregiver on their original paperwork or change to you as caregiver thru a correction/change form and 10$ to change from previous caregiver (or none named) to you as caregiver. Then you get a letter from the state informing you of acceptance as caregiver for the patient.
