Clone problems! help!

im waisting away my mothers! they are half copped up with no 0% succes rate for my clones
using a 75w hps and 3 daylight cfl.
for the medium im using "seed starters" that have "starter charge" in them.
also have a humidity dome and a heat pad to keep my babys warm.
i dont know what im doing wrong. i watched over 75 videos of cloning. i got it down to a T. but i still have not gotten one clone to form roots. this is the second time i have tried. the first time i tried. it took them 2 1/2 weeks for them to die. this time its been 5 days i cut the starter down the middle t show the tip of the clone. there are no root growing. its hard still green up intill the tip. thats were it looks like a brown/tan hard outer circal (picture number 3&4.) on the tip of the 45 degree cut i made. with no sign or roots.
what is going on?

How moist are you keeping the plugs? And you say die, what do they look like? Fallen over due to stem rot? I think I would edit out any identifiable mark on anything shown in a pic on the internet.
 
I had an old head teach me how to clone and I have about a 95% rate, and I take 12 clones every two weeks. I first fill up 12 little clear Dixie cups with soil.(I use the same soil they will later be transplanted too, in my case organic.) I then fill up a separate cup with water. I then take clones from the mother. I have heard its best to take clippings off of the bottom, but I take them from all over the plant with success from everywhere I take them. When I cut off of my mothers, I use a brand new razor blade every time. These are extremely cheap if you buy them in bulk. I make sure to cut just above a node at a 45 degree angle. I then stick them directly in the cup of water and proceed to take the rest of the clones. I then take the water cup with all 12 of my clones to my little desk setup. I have a cheap bottle of powder rooting hormone that I picked up from walmart like 2 yrs ago(shit lasts forever!). I take a pencil and make a hole in the dirt in the cups about 3/4 of the way down. I then take a clone, dip it in the powder, lightly shake it to get the excess powder off, and then put it in the soil. After doing all 12, I very lightly put a tad bit of water on the soil. Not enough to make it real wet, just moist. I then stick the cups in a small fish aquarium that has glass from a picture frame sitting on the top for a see through lid. I have a spray bottle and I mist the inside of the aquarium to raise the humidity, then I put the lid on and sit it under 4' flouros. Twice a day, I will take the lid off and gently fan fresh air into the aquarium. I then spray a 3-4 times a very fine mist into the box and quickly close the lid. After two weeks, I can see roots. This is why I use the clear Dixie cups, some say not to, but again, this is what works for me. Hope you can use some of this to be successful with your next attempt. Good luck buddy!!
 
Those are longer than I cut mine or you're not inserting them as deep. Like they said, make sure to have a node under the surface

No, just however much you need to make a correct clone. All things can be perfect, but the cut is the most important part. I'm pretty sure anytime there is enough to cut you can start cloning.
 
make sure you cut your final cut under water High room temp and too much light can cause people stress.YOU will get it soon enough. I get about 90 percent on these kushes. some strains clone in 5 to 7 days but Ive gota white fire cut thats taking 3 weeks to root
 
Ok, my 2 cents worth, I'm a n00b so take it for what it's worth...basicly everthing BongWizard said except, at the bottom of my clone I make a small slice on either side to open the hard outer layer and let the rooting powder into the soft tissue of the plant, and I use small Jiffy Garden from walyworld about 5 bucks it has a clear dome lid and expandable pellets, expand the pellets with warm PHed water, put rooting powder on the stem stick it in the pre-made hole and COVER the top with your growing medium from the pellet, you don't want any air getting in there, it will cause slow painful death to your cuttings...lol..here take a look..
