If you are going to use that many ferts you need to add in some trace elements. They help "cool" your soil by buffering the ph, and help support a diverse microbiology that can eat all of the food you are putting in the soil.
Small amounts of azomite, dolomite, gypsum, humic acid, and/or green sand will greatly benefit you. You don't need to choose all of those trace elements, but dolomite and gypsum a pretty essential in that hot of a mix. If I were using your materials I would probably amend it as such:

50% base soil
25% worm castings
10% manure (chicken is better than horse in my opinion, but if horse is all you have access to it is fine).
10% fish bone meal
2% dolomite
2% gypsum
1% epsom salt
A bit more expensive:
50% base soil
25% worm castings
5% manure (half and half chicken and horse for best results)
5% Bat guano (half and half Jamaican and Mexican for best results)
5% Fish blood meal
5% Fish bone meal
1% azomite
1% dolomite
0.5% humic acid
1% gypsum
0.5% epsom salt
1% green sand
