Doctor is very cool and mmj knowledgeable. $99 includes uploading all your documents to the state. State fee is still $150 or $75.00 if you have proof you are on food stamps or some other type of welfare.
Been there for my 3rd certification last week. Only place in Phoenix that I know of that has been there since day 1.

5) Anyone who uses the provided information for any purposes what so ever, will be assuming their own liability, and are responsible for their own actions.
6) This medicine will be consumed only by me and/or other Prop 203 patients.
7) Your generous donations are in direct relationship for the expert medical marijuana and life counseling services we provide our patients. We never accept donations in direct relationship to any costs, fee’s or any costs related to any excess medicine we might suggest or choose to share with our patients.

I see many of these same statements on the craigslist advertisements. I don't see why most would be of any benefit from a criminal standpoint other than to show that a person was trying to deal with another card holder. Who thought these up? Number 3 seems to offer a false hope of some kind. The courts ruled long ago that police can lie. An example would be a hooker
asking the under cover if he is a cop and of course the cop lies...Makes no difference. Shit there also has been at least one case of a cop with a card busting people they meet in some states.
# 7 Could go a long way in protecting you though.
