Help needed on changing or adding a provider

A freind of mine has his Script? or licance? not sure of the wording. But any how he does not have a "provider" on his licance. Not sure how to do it. He has found a provider, by doesnt know how to make the change. Thanks

That is funny, lmao. Not necessarily true though. Many expert growers do not know how to get the paper work filled out. If they can't figure it out, then I would consider someone else.

This is an area where the state could simplify everything, all they would have to do is call the bottom of the Patient Registration thr Care Giver Attestation and drop the separate form. Change forms would be on sheet, simple. VV
