Holes in a few of my top leafs........

Hellow fellow growers. Im in need of some advice. I am haveing a problem with one out of my 18 plants with a few hole on the top. I have taken pics and circled the holes in red. If anyone knows the possible cause that information would be great. I was looking on the net for people with simaler problems and one said that he was using tap water and the chlorine content was too high. He was saying if i let the tap water sit out for 24 hours it should release the chlorine. I am using tap water and the PH is right around 6.5. Im using a 600Watt HID light. Currently i have the light 23in away from my plants. When the holes occured the light was at about 19in above my plant, thats why I moved the light to 23in just in case the light was too hot. My room temp is at 75-80 Degrees. My Humidity runs from 27-45%. I am using FoxFarms Ocean Forest Blend Soil. Aslo for feeding I am usng the Budda Grow and super thrive.

The third pic is one of my plants the has been growing a little weird and I was wondering if I should be concerned.

personally, i think the plant is mutating slightly, notice how in the first pic the leaves affected are symmetrical indicating this was ever since they were tiny (unless they appeared overnight). otherwise it could be a bug problem. i don't really know so don't take me up completely, its just my guess.
 
Now you maby be right with the mutation because the holes formed at the same time as the leaves did. The new growth after the leaves with the holes looked a little haggered too. The holes also appered after I uped the dosage of there nutients. So I went back to what I was feeding befor the holes appered.
 
With a light that bright close to the plants it is possible it is a burn from a water droplet on the leaf, magnifying the light. I had some holes one time and it was a caterpillar.
Don't throw out the mutie unless you need the space for something better, they tend to grow out of it.

The caterpiller< did that affect the new growth. Because my new growth looks haggered. I have this one on its own about 5in away from my other plants just in case. I dont know if its bug because the leave b4 the holes started appering are fine and still look uneffected bye whatever the problem is. but the new growth is starting to look mutated.
