That doesn't happen in this state and probably not Washington either. California has different laws and requirements and things aren't looked into at all really.
Here in Oregon they do look so you just do have to meet the minimum requirements and I did easily.
The process was:
I went to THCF clinic and they gave me a medical release form. I gave that to my doc, they faxed my records to thcf, thcf said looks good come see our doc...
I saw the THCF nurse/then doc and she(the doc was a woman) did the recommend based on seeing me and my old medical records.
I took all the paperwork which THCF made sure I had right, straight to the state office nearby and was legal that day. However...
Months later while I'm still waiting on the actual card I get a letter saying they are confirming my condition with my regular doctor. Another month later I had my card.
I have no clue about Washington really but I just have a feeling they are probably closer to our laws, and do check. The requirements are minimal though so just meet them.
