"Honestly I'm pretty unscientific about it. I buy 35% H2O2 by the gallon, so will water it down to around 3% then pour enough in to disburse throughout the medium. I'll let it sit for a while and do it's job (sometimes overnight), then flush the media with tap water. Once it's flushed I'll do a precharge of nutes around 350ppm."

peroxide is good for root rot, if you're using 3% from the grocery store, start off at 25 ml per gallon, then drop by 5 each time till you get down to 12. you can give them 12 ml per gallon every time, without damaging the roots, and it'll keep your pots sterile. actually, once your sure the roots are good, you can start giving them 12 every other time.
i'm not sure it's roots though. i think you just over fed it badly. i'd run plain water through them the next couple of times you water, and get a little run off each time. then start feeding at 1/4 strength, every other watering. when you water in between, get a little run off, not a huge amount, but at least a quart or two
