Root rot

To begin, I have a controlled environment with all the goods. Water chiller, c02, ac unit etc... Just don't know why I keep getting the rot. I sanitize everything plus even buying all new buckets and lines along with a big air pump and stones.
Here's my current run down list of what I put in my ladies...
ML per gal
15 h202
15 cal mag
25 micro/gro/bloom
20 hygrozyme

Could it be that I'm flushing too much? I flush every 2 days with clearex at 300 ml in a 30 gl...
Any thoughts??

Ebb and Flow?
What are the pot temps?
1: during the flow?
2: when the solution isn't running?
What is your ambient air temp at pot level?
How do you sanitize?
Flushing a hydro grow every 2 days is like shooting yourself in the foot.....Why?
Try the 33% rule. Like this....
In hydro you fill your res with fresh 100% pH'ed solution...
When you go to top off the res. Add back a 33% nutrient solution. (Mix up a gallon of 100% and add 2 gallons pH'ed water to it - BINGO, 33% solution.)
Add back the 33% solution when needed, till you will have reached the total amount you filled the tank with at the start..... Now dump the res and refill with fresh 100% and do it all over again......Same for bloom or veg.
Drop the Hydrogen Peroxide.....If you have bubblers in your tank or pots,,,you don't need any H2o2.

I run a recirculating pump in my set up with temps at 69-70' at all times.. I'm running 3 raptor 8's at 700w with room temp avg 80' all day with humidity around 30-65 depending on room conditions at that time. The water level sits around 2 gallons in a 5g bucket. Can you explain to me why it's not a good idea to keep my rocks and nutes clean by flushing every other day?

Just the res has the stone.. It may be like 5 inches of water.. Keep in mind that this is a recirculating system so it's never just sitting water...
