Alright, since everyone else was so disrespectful towards what you were asking, I'll answer from personal experience.
I to was in financial need when I first got my Medical Card. I was also not 18 so I needed 2 signatures on 2 Physician Certification forms. I did some online research (You should always do this kind of research a different way, don't use the internet) and I found out that many Medical Marijuana doctors charge anywhere from 150-200$ for that Evaluation to get your Form signed. But the keywords are: Medical Marijuana. Only the special Medical Marijuana Doctors that are specially for Medical Marijuana charge that much, because if you go through them you are sure to get those forms signed.
So I turned my computer off, smoked a couple bowls, and I decided to pick up the phonebook and browse around in it. I stumbled across the Doctors Offices and Doctors section, and decided to try one from here. I found my location, and there were around 5 or 6 different locations close to me. I made a couple phone calls, and boom, I had my FREE appointment. But if you get this, there is a trick.
When you go in, make sure that you are legitimately feeling ill. (Joint Issues, Back Problems, HIV, Migraines, etc.) Say for your headache issue. Make sure you go in when you really have a headache. Honesty and legitimacy plays a key roll here. (If you are not ill, and you are lying to just get the right to grow Marijuana, exit this internet tab)
Anyways go in and tell them about your headache, (if you have an appointment tell them about that too) They will give you some medicinal attention (When I was in Middle School I was absent a day due to Migraines. I went to the hospital, and they just ran me an IV Line with some cool liquid (Forgot what it was) layed me in a bed and just told me to relax) Anyways after this is done, let them know that it is Chronic and Frequent. (I understand you suffer from that on occasion and your family has a history of them?) Tell him that you have tried everything. Tylenol, Mydol, Advil, Head-On and showers/baths but nothing helps. If he recommends you some other pill just inform him you are not big on pills.
If he hasn't already recommended or mentioned Medical Marijuana, do so yourself. Let him know that it really helps, and that your parent(s) are fine with it. (You will need there written consent/testament) If he is shaky on it because of Marijuana being Illegal, inform him that it is Legal with a Medical Marijuana card, and the only way to get that is for him to sign those papers.
Note: Don't worry if you don't get the first Doctor, or even second Doctor you try to sign it. Keep trying different Offices and Doctors, eventually you will get a bite!
Now do this at another place, get the 25$ (or 100$ if not on Medicaid) get some Medical Records along with your Parent(s) written testimony, get both Physician Certifications and put them all in an envelope. Next mail it into 'Department of Licensing and Regulatory Affairs
Michigan Medical Marihuana Registry' and hopefully within 1-3 weeks you will receive some mail with your Medical Marijuana card
Total Cost: 25$ - Medical Card Fee, maybe 10-15$ on Gas and another 1$ on Stamps.
