What is the problem? Help!

This is the second plant that has had this problem and I dont know what it is that is causing the problem!
Someone please tell me if they have any idea how to fix this! See picture. The green thing is just a twist tie holding the plant stem up.


Info:
2x3 Mylar coated grow tent
400watt metal halide and HPS
Good airflow with 2 fans running
Fox Farm Ocean Forest Soil
Nirvana Soiltabs

also try using soil with less nutrients in it.....i believe foxfarm is too hot with nutes

Looks bone dry to me. Is it that dry? Or does it just look that way? Need to keep it moist, not flooded with water just moist. I've used foxfarm with cuts and seeds and never had any problems.

The only form nutes im using are soil tabs from Nirvana
