You'll pay for it and so will the rich, most places have a national sales tax, except on essential items and quarterly rebates to poor folks, this helps with much of the cost, but not all. It would cost a third less than the current system and if you have a plan it would layer on services over the government plan in a worst case scenario. You will probably pay less in taxes than you (or your employer) does for healthcare premiums.
Single payer works pretty well everywhere else and there are a variety of systems to choose from, those that have it prefer it.
